  • Case Management services

    Case Management Services provide a single point of contact for people with a moderate level acquired brain injury to remain living at home, and their families. Case managers work one on one with clients, families and carers to identify needed services and supports for the person; offering support; advocacy; and assisting to organise and coordinate community care services. Funding may be used to provide respite, and fund social support groups.
